Terms and conditions

• Payment is required in full at each appointment. We accept cash, bank cheque, eftpos and credit card
• You acknowledge that as a courtesy you will give us at least 2 business days notice if you need to reschedule an appointment failure to do so may result in no further appointment being made available to you
• By failing to attend your appointment/s you accept full liability that any issues relating to your health and medications is solely your responsibility and not on your treating specialist
• We have the right to refuse to rebook your appointment if you have cancelled or missed 3 consecutive appointments
• You acknowledge your records may be sent to your referring doctor/hospital/health professionals involved in your care if requested

What should I bring to my first appointment?

• A referral from your General Practitioner or Specialist
• Your Medicare, Concession and your private health insurance information
• Your physical medications or top of the webster pak information
